I have already signed my encounter, but realized that I made a mistake; what should I do?
ANSWER: Mistakes are a part of life and are easily fixed in EyeCHARTS software. With a paper record, you would cross out any mistake (with a single line to leave the record readable) and simply write out the new information. We follow the exact same principle in the electronic medical record, with the added benefit of automatic time/date stamping for any changes. To make a correction, open the appropriate finalized encounter in “Edit” mode (see Fig 1 below).
INSERTING AN ADDENDUM: Once in the edit mode, you are prevented from directly changing or altering the text on the screen. You must first mark your insertion point by double clicking??
I really messed up! I accidentally used the incorrect patient when entering a new encounter. Can I delete the encounter?
ANSWER: If the encounter has not been signed, you can delete the encounter by either: (1) right-clicking on the encounter in the ENCOUNTER VIEWER screen and choosing DELETE or (2) by opening the encounter and selecting from the menu: FILE->DELETE ENCOUNTER.
If you have already signed and finalized the encounter, it is not possible to delete the encounter. In this case, create a corrected encounter for the proper patient (you can cut & paste data from the original) and use the above addendum instructions to clearly mark the incorrect encounter as void/incorrect.
